If you have been asked to serve as a godparent or sponsor for a Baptism or Confirmation, you will need to complete a Sponsor Verification Form and submit it to the parish where the Baptism or Confirmation will occur. Please download the form and complete the relevant information and send it to our parish office via mail, email (office@stclarecc.org), fax, or in-person.
After the form is received, it will be signed by the pastor and returned to you via email to the email address you have indicated on the form. Once you receive the signed form, you will need to send this to the parish where the Baptism or Confirmation will take place.
